EDUCATION AND WORK EXPERIENCE:

  • BA degree in Social Work or Psychology preferred.
  • 1-3 years progressively responsible experience in case management.
  • Demonstrates expertise in: human services, case management, community building and cooperation in congregate living environment(s), data collection/entry and reporting skillset, and time management.

  • Possession of a Washington State driver's license and has the ability to meet The Salvation Army's driving requirements
  • Ability to successfully complete The Salvation Army's Protecting the Mission training
  • Must successfully pass the background/reference check process
  • Ability and willingness to understand and fully comply with mandatory reporting requirements and laws pertaining to vulnerable adults and children
  • Complete blood borne pathogen training within 90 days of employment

SOFTWARE-RELATED SKILLS:

  • Microsoft Word, Excel, and Outlook required
  • Working knowledge of integrated database applications and ability to use new software programs with basic training

SCOPE OF POSITION:

This Case Manager position provides care for our Charis Place Program and our Pathway of Hope Program. Charis Place is a sober living program for men and Pathway of Hope is a case management program for families. The Case Manager will schedule regular meetings with program participants to assist in goal setting and successful program completion. Regular meetings with the corps officers, resident manager, and Social Services Coordinator to assess client progress will also be expected. Participation on the Charis Place Oversight Committee will be on an "as-needed" basis.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Provide intensive case management including all or part of a comprehensive assessment, case management, service authorization, information, and referral necessary for program participants to develop and pursue an effective and mutually agreed upon case plan.
  • Provide personal direct services in unusually complex situations as required by unique circumstances.
  • Provide case management for program participants with varying degrees of vocational, social, cultural and/or medical impairments hindering self-sufficiency.
  • Assess and determine employability, job readiness and/or vocational education and training needs. Create, implement, monitor, and modify case plans for achieving client economic and social self-support.
  • Provide formal and informal referral service to community resources, case management services in a multidisciplinary approach to service delivery.
  • Coordinate services with other agencies such as mental health, employment security, substance abuse programs, vocational rehabilitation, and developmental disabilities.
  • Provide formal or informal referrals to community service networks with key aging and adult community services.
  • Personal case load will include program participants who have high needs who may require intense relapse prevention support, referrals for program participants who may need psychological/psychiatric/medical evaluations and treatment.
  • Participate in weekly program house meetings, corps staff meetings, and in advanced training as determined.
  • Assist with oversight of client case records, social services systems (WellSky), HMIS, and ensure that required forms and reports are maintained in the prescribed format.
  • Prepare general correspondence as necessary.
  • Perform other administrative and operational duties as required.
  • Assist with other social service programs as needed.
  • Work with Social Services Coordinator to assess and/or involve program participants eligibility in Pathway of Hope program.

PHYSICAL REQUIREMENTS:

Physical requirements include:

  • Sit, walk, stand, bend, squat, climb, kneel and twist on an intermittent or occasional continuous basis.
  • Grasp, push, pull objects such as reference materials, files, file cabinet drawers, and reach overhead.
  • Ability to load and to change settings, devices, fixtures on various equipment used in and around an office environment.
  • Operate telephone and other electronic communication devices.
  • Operate various other office equipment including personal computers, copiers, fax machines and scanning equipment.
  • Ability to lift up to 45 lbs. occasionally
  • Ability to access and produce information from a computer
  • Ability to interpret and understand written instruction

Retirement Plans: Employer-funded Money Purchase Pension Plan (Defined Contribution Plan) 50% vested at five years eligible service time. Plus, an employee-funded voluntary 403(b) option.

Parental Leave: The benefit is 40 hours of Paid Leave for qualifying events.

Sick Leave: 12 days of Sick Leave annually accruing from day one, eligible for use after three months' service time.

Paid Vacation:

Two weeks annually, accruing from day one, for non-exempt positions.
Four weeks annually, accruing from day one, for exempt positions.
Accrued vacation is eligible for use after six months' service time.

Paid Holidays: 13 designated holidays + 1 floating holiday per year
